Our continued emphasis on quality care and patient education has led to the blending of highly trained, compassionate staffing and state-of-the-art technology to produce a multidisciplinary approach to promote wellness and independence in patients recovering from an acute illness. A feature article published by MPRO highlights the efforts to increase patient independence and decrease re-hospitalization. These programs include:

Zoe™ Fluid Status Monitoring Program

This program was designed for monitoring fluid levels by measuring the thoracic base impedance. This is a measurement of the time it takes a small frequency electrical current to travel from the top to the bottom of the thorax.

The more resistance the current meets, the drier or less fluid that exists. The less resistance that the current meets, the wetter or more fluid that exists in the chest.

Zo measurement is a quick and easy method of determining whether individuals are experiencing fluid congestion or dehydration. Research indicates that Zo is an early predictor of congestion in heart failure, showing decreases as early as two weeks prior to weight gain.

Cardiac Rehabilitation Program

This program assists post-surgical open heart patients, CHF, and other cardiovascular patients in the recovery process. This comprehensive personalized care plan incorporates patient education and an exercise program which empowers patients and their families with the knowledge of their cardiac condition, dietary needs, medical needs, and physical limits in order to prevent the need for future hospitalization.

In-Home Protime Monitoring Program

Our nurses work together with physicians and Coumadin clinics to provide in-home anticoagulant therapy monitoring. This allows our nurses to coordinate our patient’s therapy by obtaining immediate results and proper dosage adjustments while in our patient’s home eliminating the need for multiple physician office or clinic visits.

Advanced Wound Care Program

This program provides a comprehensive, in-depth wound care program that which utilizes the most up-to-date wound and skin care management products and protocols.

We have long recognized that while innovation is important, education is the key; education that can be customized to each person’s needs. We take this same approach to wound care. By devoting the necessary resources, we are able to provide a vast array of education and services to support comprehensive care plans and prevention programs.

We have a number of options available that allow us to tailor a wound care program to meet the needs of our patients, AFC facilities and caregivers. These programs, when combined with a complete line of wound care products, helps promote good wound care techniques, positive decision making and increased patient outcomes with cost-effective results.
