The Home Health Program provides Community Education seminars on health related issues to various groups within the community.

Hours:

Office hours are Monday through Friday between 8:00 a.m. and 5:00 p.m. with a 24 hour on-call nurse available.

Who is Eligible:

  • You must be under the care of a physician.
  • It must be difficult to leave your home for needed health care.
  • You must need intermittent skilled nursing care, physical therapy or Speech/language therapy.

Services Offered:

Skilled Intermittent Nursing Care

  • Instructs you on new medications
  • Assists and teaches in treatment such as tube feedings, IV infusions and nutritional counseling.
  • Checks blood pressure
  • Perform and instruct in wound care
  • Administers injections
  • Teaches family members how to care for a loved one.

Medical Social Worker

  • Assists with community resources
  • Assists with arranging alternative living arrangements (placement in Adult Foster care or Extended Care Facility)
  • Assists with long-range goal planning

Home Health Aides

  • Assists with personal care (bathing and dressing)
  • Assists with minor chores (laundry, errands and straightening the patient’s area)
  • Assists with exercise program and range of motion

Physical Therapy

  • Assists and teaches special exercise programs to increase strength, range of motion, mobility, improved general function
  • Instructs safe use of assistive devices (canes, walkers)
  • Evaluates and recommends equipment or devices to assist patients

Occupational Therapy

  • Teaches exercises to increase strength, range of motion and improve function to upper extremities
  • Evaluates and recommends equipment or devices to assist patient.

Speech Therapy

  • Evaluates and instructs regarding exercises to improve speaking, language and reasoning skills

Who Pays:

We are Medicare, Medicaid, and Blue Cross/Blue Shield (BCBS) certified, approved provider for Blue Care Network (BCN), Health Plus, Health Alliance Plan (HAP), Preferred Provider of Michigan (PPOM), and most private (health and auto) insurance's.

We bill all insurance programs directly, after any deductibles are paid. If you have no insurance coverage and are eligible, we will use an ability to pay scale.

We accept donations in honor of or in memory of your loved one. This money allows the Home Health Program to provide care for more patients.
